What is Lutetium-177 Therapy?
Lutetium-177 therapy is a form of targeted radionuclide treatment used in nuclear medicine. It combines the radioactive isotope Lutetium-177 with a carrier molecule designed to bind to a specific target on cancer cells. After binding, the Lutetium-177 component emits beta radiation that can damage tumor cells while the treatment team aims to limit unnecessary exposure to healthy tissue.
The two best-known clinical applications are Lutetium-177 PSMA therapy for selected prostate cancer patients and Lutetium-177 DOTATATE therapy for selected somatostatin receptor-positive neuroendocrine tumors.
How Does Lutetium-177 PSMA Work?
In prostate cancer, Lutetium-177 may be linked to a PSMA-targeting molecule. PSMA is a protein that is often expressed at high levels on prostate cancer cells. Before treatment, patients usually need PSMA PET/CT imaging to confirm that the tumor shows sufficient PSMA expression.
How Does Lutetium-177 DOTATATE Work?
In neuroendocrine tumors, Lutetium-177 DOTATATE targets somatostatin receptors. Patients are usually evaluated with DOTA PET/CT or other receptor-based imaging to confirm whether the tumor is suitable for receptor-targeted therapy.
Lu-177 PSMA
Used for selected metastatic prostate cancer patients after PSMA PET/CT evaluation.
Lu-177 DOTATATE
Used for selected somatostatin receptor-positive neuroendocrine tumors.
Which Patients May Be Considered?
Eligibility depends on diagnosis, stage, PET/CT findings, receptor expression, previous treatments, kidney function, liver function, blood counts, general condition and the patient’s overall oncology plan.
